Issue:
A Prescription was processed by Pharmacy for a Veteran via UPS. Pharmacy had Veteran's P. O. Box on label that was affixed on medication. During mail processing, an old physical address (different than the P.O. Box) was placed on the actual medication package that was mailed out. Prescription was delivered and signed for by the resident of the old physical address. When pharmacy was notified, staff contacted the recipient to mail the package back to the VA. When Pharmacy received the package, staff noted prescription paperwork which contained the affected Veteran's name, P. O. Box and drug information was missing.
Outcome:
The Incident Resolution Service Team has determined that the Veteran will be sent a HIPAA notification letter due to Protected Health Information (PHI) being disclosed.
