Issue:

On 8/21/14 at 3:17 p.m. BVAMC PO received a phone call and e-mail from PA letting the Privacy Office know that today Veteran A called nurse triage to let her know that PA accidentally gave him a medication sheet with Veteran B's name on it. PA immediately called Veteran A and he confirmed that he was inadvertently given a paper with Veteran B's name and information on it. Veteran A asked the PA if she minded if he shredded Veteran B's information and PA told him to go ahead and shred the paper. Veteran A stated, "OK that's what I'll do, I'll put it in the shredder." PA asked Veteran A if he would like her to mail him his medication reconciliation sheet (which PA did find lying on her desk) and he stated there was no need for PA to mail it to him as his medications had not changed. PA said that she wasn't sure how the mismatch happened but she wanted to let PO know as soon as possible. In addition, when PA called PO before e-mail was sent from PA to PO, PA advised that she called supervisor to inform her and supervisor said to contact the PO.

Outcome:

08/22/14: The Incident Resolution Service Team has determined that Veteran B will be sent a letter offering credit protection services.
