Issue:
A VA employee copied and pasted a note from Veteran B into Veteran A's chart. Then Veteran A received a copy of the medical records which had Veteran B's information in it. No Social Security Number was included in the information from the nurses intake which includes things such as medications, chief complaint, vital signs, age (no date of birth) and other nursing assessment questions. No 7332 information was involved.
Outcome:
12/03/14: The Incident Resolution Service Team has determined that Veteran B will be sent a HIPAA notification letter due to Protected Health Information (PHI) being disclosed.
