Issue:

Veteran/employee A went to the pharmacy to pick up his medications. He was only given two medications and he said his medication sheet listed four medications. He handed his medication sheet to the pharmacy tech who realized the sheet belonged to Veteran B. Veteran/employee A had not realized that he had Veteran B's medication sheet. The Pharmacy tech kept the medication sheet and reported the incident to her supervisor. Update: 09/04/12:Veteran B will receive a HIPAA letter of notification.09/10/12:Update, progress note contained full SSN and Date of birth, therefore Veteran B will recieve an offer credit protection services.

Outcome:

The Emergency Room Nurse Supervisor met with all ER Nursing staff and mandated that upon returning to the exam room with the printed discharge note, the staff again verify the patient identifiers (name and date of birth) with the Veteran, compare the response with identifiers on the note, before handing the discharge note to the Veteran.
