Issue:

Based on interview and document review, the facility failed to ensure their (PHI) Protected Health Information was not disclosed to any entity not authorized to receive the information. This failed practice resulted in unauthorized access to Patient B's demographic and medical information.Findings:On October 10, 2012, and unannounced visit was made to the facility to investigate a breach of PHI.An interview was conducted with the Compliance Specialist, on October 10, 2012, at 12:30 p.m. The Compliance Specialist stated on September 19, 2012, Patient A and B were both seen at the same physician's office. The physician wrote a prescription for Patient A. Staff printed the prescription and had the physician sign it before giving it to Patient A. The prescription contained Patient B's name and demographic information. Staff and the physician did not noticed the error prior to giving the prescription to Patient A. On September 20, 2012, Patient A noticed that another patient's name was on her prescription, and called the physician's office to report the incident. The physician's office then reported the breach to the compliance department, on September 20, 2012. The facility failed to ensure Patient B's Protected Health Information was not disclosed to any entity not authorized to receive the information.
