Issue:

Veteran suspects significant other's ex-wife (VA Employee) may have inappropriately accessed medical record and appointment information. According to Veteran, she "had several appointments inexplicably cancelled". Veteran requested SPAR (Sensitive Patient Access Report) to see if specific individual accessed her medical record. VA Employee was returned on report as having accessed Veteran's medical file on three separate occasions while Veteran was NOT an inpatient at the medical facility. Access was made on May 27, 2014 @07:59a, 09:31a; 09:31:10a using following menu options: Change Patient, Appointment Management, Routing Slips.

Outcome:

08/19/14: The Incident Resolution Service Team has determined that Veteran A will be sent a HIPAA notification letter due to Protected Health Information (PHI) being disclosed.
