Issue:
VA employee mis-faxed a request for a Home Health Agency to visit a Veteran's home. The Fax went to the general public and that person called in to let us know what she had received. VA employee has since learned they telephone number that was given to them was wrong and found the correct fax number and corrected the problem; however, a veteran's information was sent to this person. It included full SSN, Full name, home address and telephone number and also medical diagnosis information.
Outcome:
09/16/13: Veteran A will be sent a letter offering credit protection services.
