Issue:

A supervisor reports that an employee took a consult with protected health information into the restroom and left it there. When she returned to retrieve it, it could not be found. The list contained the name, date of birth, full SSN and diagnosis of 1 patient.

Outcome:

07/30/13: Due to loss of full SSN and medical information, Veteran A will be sent a letter offering credit protection services. NOTE: There were a total of 19 Mis-Handling incidents this reporting period. Because of repetition, the other 18 are not included in this report, but are included in the "Mis-Handling Incidents" count at the end of this report. In all incidents, Veterans will receive a notification letter and/or credit monitoring will be offered if appropriate.
