Issue:

On 12/17/14 at 7:31 AM, the Chief of Pharmacy emailed the Privacy Officer (PO) that on 12/15/14 the Pharmacy filled hydrocodone/acetaminophen prescriptions for Veteran A and Veteran B. Both patients received the correct medication, but Veteran A received Veteran B's paperwork in his bag in error. Veteran A called to report that he had the paperwork of the other patient in his bag. Veteran A was very upset that he now knew information concerning prescriptions that Veteran B was taking. Chief of Pharmacy said that she never personally spoke with Veteran A, but a nurse in the pain clinic conveyed to her Veteran A's concerns. The Pharmacy Chief told the nurse to ask Veteran A to mail the paperwork back to us, if he would be so kind. The Pharmacy Chief also contacted Veteran B to find out if he had Veteran A's paperwork, but Veteran B told her he never looks at any of the paperwork that is included with his prescription and he had already thrown the paperwork away. Veteran A told the nurse that he would either mail the paperwork back or bring it with him in January 2015 when he comes in for an appointment.

Outcome:

12/17/14: The Incident Resolution Service Team has determined that Veteran A will be sent a notification letter.
