Issue:

The Pharmacy Manager reported to facility Privacy Officer (PO) that a package was returned to the Pharmacy by Veteran A after it was mismailed due to mislabeling added by the mailroom. Veteran A returned package containing the label for mailing to him, however, the contents (prescription filled by Pharmacy) and other label noted on package were intended for Veteran B. Medications re-filled for Veteran A and B/corrected and fact-finding with appropriate corrective actions/remediation to be addressed.

Outcome:

03/19/15: The Incident Resolution Service Team has determined that Veteran B will be sent a HIPAA notification letter due to Protected Health Information (PHI) being disclosed.
