Issue:
Veteran A contacted Dorn File Room Supervisor by phone on 01/22/15 concerning some medical record documentation he received in the mail that did not belong to him. The records were sent by his physician. Veteran A provided the information for Veteran B via fax to the Lead Release of Information (ROI) Technician who forwarded the documents to the Health Information Management Service (HIMS) Chief for action on 01/23/15.
Outcome:
01/27/15: The Incident Resolution Service Team has determined that Veteran B will be sent a HIPAA notification letter due to Protected Health Information (PHI) being disclosed.
