Issue:
On 08/26/14, at 3:15 PM, Veteran A brought a packet into a Community Based Outpatient Clinic (CBOC) that was in with his medical information that he requested from Release of Information (ROI) Department at the Medical Center. The packet includes 6 Veterans' ROI requests that had been sent to medical center to be processed. Some of the ROI requests were date stamped and some were not. A few of them were faxed from another CBOC.
Outcome:
08/27/14: The Incident Resolution Service Team has determined that six Veterans will be sent letters offering credit protection services due to full SSN being disclosed.
