Issue:
The Dietitian inadvertently gave Veteran A his medical information during his visit today. It was found that additional Veteran information given to Veteran A also (i.e., Veteran consult listing). The Dietitian has contacted Veteran A to retrieve consult listing. The list contained 35 other Veterans' last four SSN and name.
Outcome:
10/01/14: The Incident Resolution Service Team has determined that thirty-five (35) Veterans will be sent HIPAA notification letters due to Protected Health Information (PHI) being disclosed.
