Issue:

Veteran B returned a package to the facility Fee-Basis Section noting that information contained in VA mailed envelope had been incorrectly sent to Veteran A (envelope contained information on Veteran B but was mismailed to Veteran A. Original mailed package was returned with all documents to the facility - but had been opened and handwritten note inside indicated that the Veterans (A and B) had communicated with each other and decided to return the envelopes to the VAMC. Mismailing and returned packet immediately reported to the facility Privacy Officer by the Supervisor of Fee-Based Section and fact-finding was initiated.

Outcome:

12/10/13: Veteran B will be sent a letter offering credit protection services.
