Veteran A was admitted under Veteran B's identity on accident. Names are the same, dates of birth are remarkably similar in month and day. Veteran A received Veteran B's wristband during admission with full SSN, name, DOB. Veteran A was inpatient entire time until being discharged, today. Records have been rectified, but Privacy notified, now.
Outcome:
01/15/15: The Incident Resolution Service Team has determined that Veteran B will be sent a letter offering credit protection services.
