A Veteran called the Privacy Officer to let her know that he received another Veterans lab results in the mail. The Privacy Officer asked the Veteran to bring the information back on his next appointment which is 11/04/14.
Outcome:
11/03/14: The Incident Resolution Service Team has determined that Veteran B will be sent a HIPAA notification letter due to Protected Health Information (PHI) being disclosed.
