Issue:
Veteran A reported to the pharmacy with a package he received in the mail. The package contain medication for veteran B. The package contained home medications for veteran B that was collected when veteran B became a inpatient. The pharmacy mislabeled the medication with veteran A name and mailed it to him. Update: 02/01/11:Veteran B will be sent a letter of notification.
