Veteran A presented Dr's orders at the pharmacy window from the ER dept. When veteran A was given his Dr's orders he was also given documentation of veteran B. The Pharmacy kept veteran B's documentation. Update: 02/01/11:Veteran B's full name, ssn, and medications were on the documentation. Veteran B will be sent an offer for credit protection services.
