Narcotic's inspector sent a list through the interoffice envelope and this information did not reach the destination. This list had 6-8 patients on the list which included the full name and medications.

09/25/14: The Incident Resolution Service Team has determined that Veteran B will be sent a HIPAA notification letter due to Protected Health Information (PHI) being disclosed.
