Veteran A received his test results letter and there were letters in his envelope for 4 other Veterans. Veteran A returned all letters to the Privacy Officer (PO). The letters all had Veterans' names, addresses, tests results and diagnoses.
Outcome:
06/11/15: The Incident Resolution Service Team has determined that four Veterans will be sent a HIPAA notification letter due to Protected Health Information (PHI) being disclosed.
