Issue:
Based on interview and record review, the facility failed to ensure staff register patients (Patient A) to the correct physician, and fax the patient's lab results to the correct physician's office. Findings:During a record review conducted for Patient A, documentation indicated a one-page laboratory results document with the patient's name, date of birth, account number, and medical record number was faxed to the incorrect physician's office. On March 8, 2013, at 9:40 a.m., an interview was conducted with the Executive Secretary. She stated, "Patient A came to the lab. Staff registered the patient under a physician other than the patient's PCP (Primary Care Physician). The lab results were faxed to the wrong physician's office."
