Based on interview and record review, the facility failed to ensure a patient's (Patient A) PHI (protected healthcare information) was protected. Patient B was registered under Patient A's information.Findings:On February 8, 2013, a record review was conducted. The documents titled, "Conditions of Admission," was stamped with a Patient A's name, the date of birth, a medical record number and account number, and placed in the record for Patient B, who had the same last name as Patient A.In an interview with the Executive Secretary, she stated, "The patient who was a minor was registered under another patient with the same name, but different date of birth. The error was found before discharge from the emergency room."
