Issue:
Veteran A presented as a walk-in patient to a provider covering for his regular provider. The provider handed Veteran A transfer paperwork that belonged to Veteran B. Both patient A and B have the same last name, but different last four SSNs. Veteran A's adult son subsequently notified the facility of the error. At this point we have not been able to contact the adult son to return the paperwork to the hospital. His desire was to retain custody until he could verify his father's information was correct in CPRS.
Outcome:
03/26/14: The Incident Resolution Team determined that Veteran A will receive a letter offering credit protection services since his full SSN was compromised.
