Issue:

Generic letters were sent to Veterans informing them of a new Community Based Outpatient Clinic (CBOC) opening. One batch of letters were duplexed and sent, resulting in letters sent to Veterans with another Veterans letter containing full name and address on the back side.

Outcome:

06/03/14: It was a batch of 1000 letters, 500 of which received a letter with the duplicated letter on the back side. The Director of Medical Media is trying to determine which 500 did not receive a "CBOC Grand Opening" letter. The Privacy Officer (PO) believes he can because he has received some back from Veterans reporting. It was all the even numbers that did not receive the letter so 500 total. 06/05/14: The PO spoke with the Medical Media Director. The names and addresses were pulled from Vista and it was just the Veterans who are enrolled. That makes the names and addresses protected health information (PHI) and will require 500 HIPAA notification letters, a press release and web posting. The Incident Resolution Team advised the PO that the Director, Incident Resolution Service, will need the drafts of the letters and press release NLT 06/30/14. 06/23/14: The letters and press release were approved by the Director, Incident Resolution Service. 07/02/14: The letters are planned to be mailed on 07/07/14 and the press release and web posting are scheduled for 07/09/14. 07/09/14: The letters went out yesterday.
