Issue:

A Veteran went to the Pharmacy to pick up his prescription and found out that the technician handed his medication to another Veteran with the same last name. The Pharmacy called this Veteran at home and asked him to bring the medication back to the facility. The medications was returned and the correct medication was given to the Veteran. The medication was appropriately destroyed.

Outcome:

10/06/14: The Incident Resolution Service Team has determined that Veteran B will be sent a HIPAA notification letter due to Protected Health Information (PHI) being disclosed.
