Research coordinator failed to have a subject sign the HIPAA authorization prior to collecting blood and urine, and then sending it to an outside medical center for analysis. Also provided Name and address to nonprofit for payment to the subject.

Outcome:

12/12/14: The Incident Resolution Service Team has determined that the Veteran will be sent a notification letter, since this was an unauthorized disclosure. The Veteran will be asked to sign the HIPAA consent form.
