Issue:

A Veteran presented to the SAC phlebotomy area to be drawn for his INR. He stated that he found out that he need to fast from another Employee via Facebook. The employee asked him for his last name and last four via Facebook. Then she proceed to look up the Veteran's information and told him that he needed to fast. This information was shared with the Veteran via Facebook. The Supervisor of this employee is investigating these allegations. This employee is not supposed to review records unless the patient is there for blood draw.

Outcome:

10/09/14: The Incident Resolution Service Team has determined the Veteran will be sent a HIPAA notification letter due to Protected Health Information (PHI) being disclosed.
