Issue:

While transporting some Beneficiary Travel documents from the cashiers cage to the Allergy Clinic, the employee fell outside of the Medical Center. In doing so, the employee dropped the documents and has been unable to find one of them. This document contains Veteran A's name, full social security number, date of birth and possible appointment information. Update: 10/26/11:Veteran A will be sent a letter offering credit protection services.NOTE: There were a total of 21 Mis-Handling incidents this reporting period. Because of repetition, the other 20 are not included in this report, but are included in the "Mis-Handling Incidents" count at the end of this report. In all incidents, Veterans will receive a notification letter and/or credit monitoring will be offered if appropriate.

Outcome:

Information will be transported in a zipped and locked bag. All employees were educated on the new procedure.
