Issue:
On 06/18/14, the Privacy Officer (PO) was visited by the Lead Release of Information (ROI) Clerk who reported that an ROI Clerk was visited by Veteran A on 06/18/14. Veteran A turned in three documents that he received in the mail that pertained to Veterans B, C, and D. All three documents were dated 06/06/14. The documents were as follows: Lab results for Veteran B, showing his full name, home address, full SSN, and date of birth. Radiology Exam results for Veteran C, showing his full name and home address. Lab results for Veteran D, showing his full name and home address.
Outcome:
06/18/14: The Incident Resolution Service Team determined that Veteran A will receive a letter offering credit protection services since his full SSN was compromised. Veterans C and D will receive a HIPAA letter of notification.
