Issue:

Patient B's information was inappropriately scanned into Patient A's medical record. As a result, Patient B's information was mistakenly released to Patient A's attorney upon request for date-range of records. The Privacy Officer spoke with the attorney, and no further disclosure has occurred . They will send back Patient B's medical records to be destroyed. Patient B's records have also been eradicated from Patient A's record and appropriately scanned into Patient B's record. This was discovered by a VHA employee in Cardiology Service that was looking through Patient A's records, and told Patient A to discuss with Privacy Officer.

Outcome:

02/10/14: Patient B will be sent a letter offering credit protection services.
