Patient A's bag of medications was mailed to Patient B. Patient A contacted the Pharmacy and informed them of the error. He is sending the medications and packaging back. Once the packaging is returned, the Pharmacy should be able to tell how this error occurred. Update: 03/27/12:Patient B will be sent a notification letter due to name and Protected Health Information being disclosed.
