Issue:
On January 6, 2015, we mailed a correspondence to Veteran A. While reviewing the evidence within Veteran A file, an employee noticed thet the letter contained the complete name and Social Security Number of Veteran B. We have not been able to contact Veteran A to determine what happened to the initial letter.
Outcome:
02/10/15: The Incident Resolution Service Team has determined that Veteran B will be sent a letter offering credit protection services.
