Issue:
On 02/26/14, Veteran A contacted the Privacy Officer (PO), Nashville Campus, regarding the following concern: On 01/29/14, Veteran A was seen in Orthopedic Clinic, Nashville Campus. At check-out, Veteran A was given a future appointment letter by the Clerk. Today, Veteran A viewed the appointment letter and noted it belonged to Veteran B. Veteran Bs personal identifiable information includes: full name, last four social security number, medical condition, and appointments. Arrangements were made for Veteran A to return Veteran Bs appointment letter to the PO, Nashville Campus.
Outcome:
02/27/14: The Incident Resolution Team (IRT) determined that Veteran B will receive a HIPAA letter of notification since protected health information (PHI)was exposed.
