Pharmacy Service was contacted by telephone by Veteran A, who was reporting non-receipt of his medication which was sent on 02/18/14. Upon Pharmacy Service initiating a UPS tracking search, it was found that receipt signature was that of Veteran B on 02/19/14. Pharmacy Service is still attempting to make contact with Veteran B to confirm receipt of the incorrect medication and arrange for return of that medication. Information contained on the mis-mailed medication of Veteran A was: Full Patient Name Patient Address Name of Provider Medication Information
Outcome:
02/26/14: The Incident resolution team (IRT) determined that Veteran A will receive a HIPAA letter of notification.
