Issue:

Ongoing OIG investigation. OIG Investigator reported an Excel file with Tampa VA patients that were found offsite and preliminary indication are that information was being used for tax fraud.

Outcome:

11/26/13: The OIG has said the spreadsheet contained names, SSN's and dates of birth of 49 Veterans and has provided the Tampa VAMC with the details. The OIG has given the OK to notify the Veterans. The OIG reports that this is a stand-alone incident and only the 49 Veterans are involved. Five of the Veterans are deceased. The 44 living Veterans will be offered credit protection services and the five deceased Veterans' next-of-kin will be sent letters of notification.
