Issue:
A Veteran/Employee had called the business office manager to let her know that he needed to go to the employee health because he was sick. He went to employee health and employee health sent him home. A lead clerk called to tell the business office manager that employee health sent him home and the business office manager stated that he wondered if he was seen as employee health or as a Veteran. She did not mean for the lead clerk to look in his record, but she did. The lead clerk got into to see if he was seen as employee health or as a Veteran. The business office manager told the lead clerk that they cannot look into the Veteran/employee record for this information. Then the lead clerk called the employee and confessed what she did and the employee said there was no problem under the circumstances and he was find with that.
Outcome:
04/22/14: The Incident Resolution Team has determined that there was a policy violation. While there was an unauthorized access/disclosure of data, it has been determined that the incident has a low probability of a risk of compromise.
