Issue:

Veteran A reported to facility Privacy Officer (PO) that he had received a phone call yesterday from unknown male (heavy Hispanic accent) from Colorado, telling Veteran A that he had just received a packet of medical record information that was not his and he was concerned about it and was putting it back in the mail to forward the packet and make sure that Veteran A received information packet. Veteran B, who received this mis-mailed packet, noted that there was time-sensitive information in the packet and information regarding a referral for non-VA care. At this time, the packet has been reportedly mailed back to Veteran A per communication by phone from Veteran B. Veteran A reported his concerns and above telephone conversation to facility PO, and will bring packet in for review/fact-finding upon receipt at home address. Ticket entered for initial fact-finding per internal processing at facility and await further information/update from Veteran A once the mailed/forwarded packet is received at his home address.

Outcome:

04/16/15: The Incident Resolution Service Team has determined that Veteran A will be sent a letter offering credit protection services.
