Issue:
Based on interview and record review, the hospital failed to ensure that Patient 1's protected health information was kept confidential from anyone not authorized access.Findings:An interview with the Health Information Manager (HIM) was conducted on 7/2/12 at 1:10 P.M. The HIM stated that a health information staff processed two requests for two different patients, Patient 1 and another patient, at the same time. The health information staff inadvertently faxed Patient 1's medical record to requesting agency for the other patient. The medical record that were inadvertently faxed to the wrong agency indicated Patient 1's name, date of birth, medical record number, medical history, lab results, diagnoses, medications, test results, and radiology results.
