Based on interview and record review, the hospital failed to ensure that Patient 1's protected health information (PHI) was kept confidential from anyone not authorized access.Findings:An interview with the hospital's Patient Access Service Manager (PASM) was conducted on 7/2/12 at 1:30 P.M. The PASM stated that an emergency room staff inadvertently gave Patient 1's facesheet to another patient. The facesheet indicated Patient 1's name, date of birth, home address, phone number, insurance information, and diagnosis.
