Issue:
Veteran B received a copy of Veteran A's document titled Application for Emergency Detention of Mentally Ill and Dangerous Person. Copy was with Veteran B's clinic discharge instructions. Veteran B visited the Clinic Friday afternoon 7/25, realized on Sunday 7/27 that not all documents belonged to him. Veteran B returned the documents to the nearest VA Clinic on Monday 7/28.
Outcome:
07/28/14: The Incident Resolution Service Team has determined that Veteran A will be sent a letter offering credit protection services.
