Issue:

A pharmacist placed Morphine in the wrong prescription bag when completing the filling process. Pharmacy was notified of the error on 3/31/14 and patient was called in to discuss the error. Patient stated he had taken all of the morphine, with no adverse effects. Patient discarded the bottle when he was through with the tablets. Appears the vault pharmacist did not check the name on the bottle and the name on the bag twice before completing the prescription order.

Outcome:

04/18/14: The Incident Resolution Team has determined that the Veteran whose name and medication type were disclosed will be sent a HIPAA notification letter due to Protected Health Information (PHI) being disclosed.
