Issue:

A Nurse at VA Illiana Health Care System Peoria Community Based Outpatient Clinic (CBOC) posted a photo with sensitive patient information onto her personal Facebook page. In viewing the photo, the employee's workspace is clearly visible with a stuffed animal gift as the centered object and assumed purpose of the photo, but in the lower part of the photo, the day's list of scheduled patients is visible. This list includes patient names and SSN. By zooming in on the photo it is difficult to discern the characters/text from the document, so it is unclear if the patients' information has been compromised.

Outcome:

06/09/14: There are 6 individuals whose information is potentially visible on the image. Using basic photo viewing software, the only legible information appears to be the words "test date" on two of the lines and portions of the test date. 06/17/14: The DBCT decided that this is low risk of compromise and can come off the report.
