A Veteran's prescription was mailed to a very similar address and was signed for by an illegible signature. The residents of the incorrect address claim no knowledge of the prescription.
Outcome:
03/27/15: The Incident Resolution Service Team has determined that the Veteran will be sent a HIPAA notification letter due to Protected Health Information (PHI) being disclosed.
