Issue:
On 08/08/14, BVAMC Associate Chief of Pharmacy emailed the Privacy Officer (PO), Chief of Pharmacy, Pharmacy Secretary, and Quad that on 08/07/14 a prescription and accompanying paperwork intended for Veteran A was dispensed in error to Veteran B at the Pharmacy window at the BVAMC. Veteran B looked at the package later that evening and realized that it was intended for Veteran A. Veteran B called to report the error on 08/08/14. Veteran B will bring the package (prescription and accompanying paperwork) intended for Veteran A back to the medical center on 08/09/14.
Outcome:
08/11/14: The Incident Resolution Service Team has determined that Veteran B will be sent a letter offering credit protection services.
