Issue:
A Supervisor brought to the Privacy Officer a possible breach on a Veteran/Employee who had expired. It was stated that there might be several people looking into the Veteran/Employees record inappropriately. The Privacy Officer is investigating the allegations.
Outcome:
05/09/14: After pulling the Sensitive Access list which 6 employees had entered this record inapproprately - 2 of these employees were from CPAC, so the information was forwarded to the appropriate Supervisor. All Supervisors of these employees were informed and referred to Human Resourses for approprate steps to be taken on all of these employees who accessed the records of this deceased Employee/Veteran's record. 05/12/14: The Incident Resolution Team determined that the deceased Veteran/Employee's next of kin (NOK) will receive a NOK letter.
