Issue:

Veteran A came into our office to report that he received medical documents that were supposed to be returned to Veteran B. Veteran A brought in one of the documents, but the rest are still an his custody. The document that Veteran A returned contained the following information: Veteran B's emergency room discharge instruction, name, and date of birth.

Outcome:

02/05/14: Veteran B will receive a letter offering credit protection services. OIG is sending someone over to Veteran A's house to recover our documents. The Privacy Officer (PO) from the Regional Office reviewed Veteran A's hard copy record. He is going to review Veteran B's paperless record this afternoon. He said that when the Veteran sends in documents they are scanned into the record and the employee who scanned them may have attached Veteran B's documents to Veteran's letter. She has been counseled to be more careful. The PO pulled Veteran B's medical records from Veteran A's file. OIG paid him a visit but he wasn't home. They will try and reach out to him again. Later he called the PO to say that he will return the documents tomorrow. 02/07/14: OIG was able to retrieve Veteran B's records. Veteran A had given Veteran B's records to the Assistant Associate Director at the VA Medical Center.
