Issue:

Veteran A received Veteran B's medical records in the mail. Veteran A had previously requested a copy of his file, which was mailed on a previous occasion. This second package arrived with no notice, and with Veteran B's files inside a package from Tampa VAMC. The files were recovered by an in-home VA care team. The care team has made arrangements to provide the package to the facility Privacy Officer (PO) for follow-up investigation. Initial report indicates the package originated from Tampa VAMC.

Outcome:

06/21/13: Veteran B will be sent a letter offering credit protection services, as his full SSN and other information were disclosed.
