Issue:
Veteran A received a letter addressed to Veteran B. The letter contained Veteran A's, name, SSN, and address. The information in the body of the letter belonged to Veteran B. The letter contained Veteran B's name only, along with an explanation as to why the VA forgot to process his claim for hypertension. The letter also listed some additional medical condition claimed by Veteran B. Veteran A returned the letter to the Regional Office.
Outcome:
05/22/13: Veteran B will be sent a general notification letter due to Protected Health Information (PHI) being disclosed.
