Issue:
Veteran called stating that he had received form 10-5345 ROI. The addressed envelope was correct and sent to him. However, on the inside form, was another Veteran's name and phone number. He stated that he felt the mishap occurred that his last four of SSN were the same as the identified Veteran's four digit phone number. He is returning the form to our CBOC. PO reviewed the record and have contacted the employee. The Veteran requested copies of his medical record and the employee had sent him the 10-5345 to sign. When it is returned I will have the details of the information disclosed to the wrong Veteran. The employee placed a new form in the mail to the right Veteran.
Outcome:
11/06/14: The Incident Resolution Service Team has determined that the Veteran will be sent a HIPAA notification letter.
