Issue:
As reported by the Medical Center Veteran A received a prescription belonging to Veteran B. He used that medication for 3 days but reported no ill effects to the Medical Center. Initial information provided by the facility was incorrect so further investigation revealed that the medication from the Dallas CMOP in May 2011 was received in error. Both Veterans had medication packed on same day and at the same time in May. It appears the item was left out of the original package (although initially scanned correctly) and placed in Veteran As package in error. Veteran A returned the medication to the Beaumont Medical Facility. Veteran B's name and type of medication was compromised. The packer will be counseled and retrained in proper packing procedures. Update: 07/21/11:Veteran B will receive a letter of notification.
